Polis has launched The Diabetes Impact Project (DIP-IN) Community Profiles about the three focus neighborhoods involved in this project: Northeast, Northwest, and Near West. These community dashboards empower communities and community health workers to understand the context of their neighborhood. Data in the dashboards relate to the DIP-IN project goals of examining community-level risk factors, access to care, and the physical and social environment. This initiative led by the Richard M. Fairbanks School of Public Health builds resources and connections within communities of Indianapolis with the goal of preventing diabetes or better managing it so that people can live long and healthy lives.
